Description

This form is also commonly called "The Green Form" because it has a wide green band across the top.  Although it seems you can also download it from the internet, so it may not always have that green band on it.

It is not our intent to show you how to get a divorce by mutual consent.  In fact, this may not be legal in some countries as both parties do not have to be present when submitting the form.  So there are no blank forms for this here.  But since your spouse may ask you to sign one, we feel you should know what the form looks like and understand the contents.  So we do provide an English translation and annotated form.

Someday we may add a more readable and complete English translation here, like the other forms, but it is not a high priority.  If you would like to submit the text electronically, we will be happy to add it here.
